21px">落实食品安全主体责任,保障广大消费者饮食安全,根据《中华人民共和国食品安全法》《网络餐饮服务食品安全监督管理办法》等法律法规</span><span style="font-family: 仿宋_GB2312;font-size: 21px">和省、市相关</span><span style="font-family: 仿宋_GB2312;font-size: 21px">规定</span><span style="font-family: 仿宋_GB2312;font-size: 21px">,我局决定在全县范围内开展</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位食品</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">安全</span><span style="font-family:仿宋_GB2312">“红黑榜”评定及</span></span><span style="font-family: 仿宋_GB2312;font-size: 21px">公示</span><span style="font-family: 仿宋_GB2312;font-size: 21px">工作。现将有关事项通告如下:</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 黑体;font-size: 21px">一、评定范围</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">及提供者</span><span style="font-family: 仿宋_GB2312;font-size: 21px">,包括</span><span style="font-family: 仿宋_GB2312;font-size: 21px">通过网络订餐平台提供餐饮服务的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务</span><span style="font-family: 仿宋_GB2312;font-size: 21px">单位及提供者</span><span style="font-family: 仿宋_GB2312;font-size: 21px">。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 黑体;font-size: 21px">二、评定标准</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 楷体_GB2312;font-size: 21px"><span style="font-family:楷体_GB2312">(一)</span><span style="font-family:楷体_GB2312">“红榜”评定标准</span></span></strong></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 仿宋_GB2312;font-size: 21px">符合下列</span></strong><strong><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">情形的,将列入</span><span style="font-family:仿宋_GB2312">“</span></span></strong><strong><span style="font-family: 仿宋_GB2312;font-size: 21px">红</span></strong><strong><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">榜</span><span style="font-family:仿宋_GB2312">”:</span></span></strong></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">1.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">持有效《食品经营许可证》</span><span style="font-family: 仿宋_GB2312;font-size: 21px">或《餐饮备案证》</span><span style="font-family: 仿宋_GB2312;font-size: 21px">,并在</span><span style="font-family: 仿宋_GB2312;font-size: 21px">经营场所显著位置</span><span style="font-family: 仿宋_GB2312;font-size: 21px">依法依规进行公示,</span><span style="font-family: 仿宋_GB2312;font-size: 21px">网络餐饮服务单位还应当在</span><span style="font-family: 仿宋_GB2312;font-size: 21px">网络订餐平台公示完整、准确、真实</span><span style="font-family: 仿宋_GB2312;font-size: 21px">的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">信息</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">2.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">加工制作场所环境整洁,布局合理,功能区划分明确,食品加工操作规范,严格遵守食品安全操作规范加工制作食品,无交叉污染现象</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">3.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">食品原料采购渠道正规,严格执行索证索票和进货查验制度,能提供完整的食品原料采购凭证、检验检疫合格证明等资料,确保食品原料可追溯</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">4.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">厨房设施设备齐全,性能良好,定期维护保养,能够满足食品加工制作要求,如冷藏、冷冻、消毒等设备正常运行且温度达标</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">5.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮具清洗消毒严格按照规定程序进行,配备充足的消毒设施设备,有专门的餐饮具清洗消毒区域,消毒后的餐饮具符合食品安全标准,并按要求存放</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">6.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">从业人员持有有效健康证明,个人卫生良好,</span><span style="font-family: 仿宋_GB2312;font-size: 21px">穿戴整洁的工作衣帽,规范佩戴口罩进行食品加工制作活动。</span><span style="font-family: 仿宋_GB2312;font-size: 21px">定期参加食品安全培训,熟悉食品安全相关法律法规和操作规范知识,能熟练掌握本岗位食品安全操作技能</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">7.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">近一年内未因食品安全问题受到行政处罚,且在网络订餐平台上消费者评价较好,投诉举报较少,投诉举报处理及时、有效,消费者满意度较高。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 楷体_GB2312;font-size: 21px"><span style="font-family:楷体_GB2312">(二)</span><span style="font-family:楷体_GB2312">“黑榜”评定标准</span></span></strong></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">存在下列情形之一的,将列入</span><span style="font-family:仿宋_GB2312">“黑榜”:</span></span></strong></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">1.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">未取得食品经营许可证</span><span style="font-family: 仿宋_GB2312;font-size: 21px">(餐饮备案证)</span><span style="font-family: 仿宋_GB2312;font-size: 21px">或超出许可经营范围从事网络餐饮服务活动,或食品经营许可证</span><span style="font-family: 仿宋_GB2312;font-size: 21px">(餐饮备案证)</span><span style="font-family: 仿宋_GB2312;font-size: 21px">未在网络订餐平台显著位置公示的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">2.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">加工制作场所环境脏乱差,存在严重卫生死角,如地面、墙壁、天花板有大量油污、灰尘、霉斑等,垃圾未及时清理,严重影响食品安全的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">3.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">食品原料采购把关不严,使用来源不明、过期变质、无检验检疫合格证明或其他不符合食品安全标准的食品原料加工食品,存在严重食品安全隐患的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">4.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">食品加工操作过程不规范,生熟食品未分开存放、加工,加工过程中存在交叉污染现象,如使用同一工具、容器处理生熟食品等</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">5.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">厨房设施设备缺失或损坏严重,无法正常运行,如冷藏、冷冻设备不能正常制冷,消毒设备无法使用等,不能保证食品加工制作和储存条件符合食品安全要求的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">6.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮具清洗消毒不规范,未配备餐饮具清洗消毒设施设备或未按规定程序进行清洗消毒,导致餐饮具表面有明显污渍、残留食物残渣或微生物超标等情况,对消费者健康构成威胁的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">7.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">从业人员未取得有效健康证明上岗工作,或个人卫生习惯差,如未穿戴工作服、帽子、口罩,在食品加工制作区域内吸烟、吐痰、乱扔垃圾等</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">8.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">近一年内因食品安全问题受到两次及以上行政处罚,或因食品安全问题被消费者多次投诉举报且查证属实,未及时整改或整改不到位的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">;</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">9.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">经消费者投诉举报查实或</span><span style="font-family: 仿宋_GB2312;font-size: 21px">违反</span><span style="font-family: 仿宋_GB2312;font-size: 21px">相关</span><span style="font-family: 仿宋_GB2312;font-size: 21px">法律法规和要求的</span><span style="font-family: 仿宋_GB2312;font-size: 21px">其他</span><span style="font-family: 仿宋_GB2312;font-size: 21px">行为,市场监管部门认为符合列入黑榜的情形。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 黑体;font-size: 21px">三、评定程序</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 21px">)</span><span style="font-family: 仿宋_GB2312;font-size: 21px">信息采集:</span><span style="font-family: 仿宋_GB2312;font-size: 21px">由</span><span style="font-family: 仿宋_GB2312;font-size: 21px">市场监督管理局通过日常监督检查、专项检查、</span><span style="font-family: 仿宋_GB2312;font-size: 21px">随机抽查</span><span style="font-family: 仿宋_GB2312;font-size: 21px">、投诉举报处理、网络监测等途径</span><span style="font-family: 仿宋_GB2312;font-size: 21px">,</span><span style="font-family: 仿宋_GB2312;font-size: 21px">收集</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">的食品安全相关信息,作为</span><span style="font-family:仿宋_GB2312">“红黑榜”评定的依据。</span></span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">(</span><span style="font-family: 仿宋_GB2312;font-size: 21px">二</span><span style="font-family: 仿宋_GB2312;font-size: 21px">)信息</span><span style="font-family: 仿宋_GB2312;font-size: 21px">审核:对采集到的信息进行整理</span><span style="font-family: 仿宋_GB2312;font-size: 21px">后</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">,确定拟列入</span><span style="font-family:仿宋_GB2312">“红黑榜”的</span></span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位</span><span style="font-family: 仿宋_GB2312;font-size: 21px">名单</span><span style="font-family: 仿宋_GB2312;font-size: 21px">并进行公示</span><span style="font-family: 仿宋_GB2312;font-size: 21px">四、管理措施</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 楷体_GB2312;font-size: 21px"><span 仿宋_GB2312;font-size: 21px">在</span><span style="font-family: 仿宋_GB2312;font-size: 21px">有关</span><span style="font-family: 仿宋_GB2312;font-size: 21px">媒体平台进行宣传表扬,树立行业正面典型,提高其知名度和美誉度。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">2.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">减少日常监督检查频次,但在重大节假日、食品安全专项整治等时期仍进行必要的监督检查,确保食品安全状况持续良好。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><strong><span style="font-family: 楷体_GB2312;font-size: 21px"><span style="font-family:楷体_GB2312">(二)对列入</span><span style="font-family:楷体_GB2312">“黑榜”的</span></span></strong><strong><span style="font-family: 楷体_GB2312;font-size: 21px">餐饮服务单位</span></strong></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">1.</span><span style="font-family: 仿宋_GB2312;font-size: 21px">列入重点监管对象,加大监督检查</span><span style="font-family: 仿宋_GB2312;font-size: 21px">和抽检</span><span style="font-family: 仿宋_GB2312;font-size: 21px">频次</span><span style="font-family: 仿宋_GB2312;font-size: 21px">。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">2.责令餐饮服务单位限期整改,整改期满后,由市场监管部门现场验收,拒不改正或整改不合格地按照程序查处。对网络餐饮服务提供者,将向相关网络订餐平台通报情况,要求平台采取限制流量、调整排名、暂停上线等措施,督促其进一步整改。</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 黑体;font-size: 21px">五、其他事项</span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span style="font-family: 仿宋_GB2312;font-size: 21px">“红黑榜”</span><span style="font-family: 仿宋_GB2312;font-size: 21px">将</span><span style="font-family: 仿宋_GB2312;font-size: 21px">实行动态管理,根据</span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">食品安全状况的变化适时进行调整。已列入</span><span style="font-family:仿宋_GB2312">“红榜”的</span></span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">,如出现食品安全问题或不符合</span><span style="font-family:仿宋_GB2312">“红榜”评定标准的情形,将从“红榜”中移除;列入“黑榜”的</span></span><span style="font-family: 仿宋_GB2312;font-size: 21px">餐饮服务单位</span><span style="font-family: 仿宋_GB2312;font-size: 21px">,在完成整改并经市场监督管理</span><span style="font-family: 仿宋_GB2312;font-size: 21px">部门</span><span style="font-family: 仿宋_GB2312;font-size: 21px"><span style="font-family:仿宋_GB2312">验收合格后,可从</span><span style="font-family:仿宋_GB2312">“黑榜”中移除,但在一定时期内将作为重点监管对象加强监管。</span></span></p><p style=";text-align: justify;font-family: Calibri;font-size: 14px;text-indent: 43px;line-height: 37px"><span
